Plugin for Display Resolution and Refresh Rate

I am looking for a plugin that allows me to output the display resolution as well as the refresh rate of an ATI 5xxxx series Radeon video card. I have found a solution for the resolution part, but thus far came up empty for the current display refresh rate, e.g. 60 Hz or 24 Hz. So I am looking to output something like '1920x1080 @ 24Hz'. Something like '1080p60' or '1080p24' would also do.
Anybody who can help?
