I am having problems getting this combination working. I have tried (with version 5.4) adding "LPT1", "$278", "$378", and "9C00" to no avail. I have pictures of the card config.

Try $9C00 or $9800 as the port address (with the $). There are a couple of other posts on the forum about PCI parallel cards and the address seemed to be the problem.
